  7. Did you weigh yourself and write down your weight this week?

  8. Have you reduced the amount of alcohol you’re consuming?

  9. If you smoke-have you thought of the health consequences, and taken any steps to stop, or get help?

  10. Have you thought about how you can reduce the stress in your life?
